Direct Deposit Form
- Use this form if you would like to update the account for your RubiconMD direct deposits
Both of these forms can be completed electronically with the links provided and can be
updated at any point. Once they are completed they go directly to our finance and
accounting team for review. They will reach out if they have any questions.
Important Notes
Changes will be reflected in the following month, no changes will be reflected during the
month they were submitted. Example: If you update your Direct Deposit form on December
5th, you will not receive payment into that account till your January payment.
Keeping your name and address up to date with the W9 form is incredibly important for tax
season. We mail our 1099s and do not send out automatic digital copies. Please consult
your tax advisor for questions regarding changes to an LLC and/or a personal account.
