Keeping your Medallion profile current is crucial for ensuring Oak Street Health consults are appropriately assigned through State-Based Priority routing. Follow these steps on how to manage your licensure data.
1. Accessing Your Medallion Profile
Navigate to app.medallion.co and log in with your registered email address and password. If you don’t have these credentials, please follow the Forgot Password instructions on the webpage before proceeding.
2. Adding a New State License
If you have acquired a license in a new state, you must add it to your Medallion profile to start receiving Oak Street Health consults for that region.
- Go to Licenses → Existing Licenses
- Click "Add Existing License"
- Complete the required fields:
- State: Select the issuing state from the dropdown menu.
- License Number: Enter the exact alphanumeric license number.
- Issue Date: Provide the original date of issuance.
- Expiration Date: Provide the date the license is set to expire.
- Upload: Upload a copy of the state license
- Click "Save".
3. Updating an Existing or Renewed License
When you renew a license, you must update the expiration date in Medallion and attach new confirming documentation in order to continue receiving Oak Street Health consults in that region.
- Go to Licenses → Existing Licenses
- Find the license, hover on the three dots at the end of that row, and click on "Edit" (pencil icon).
- Update the Expiration Date field to reflect your new renewal date.
- Upload your new proof of renewal in the document upload section.
- Make sure the Status field is set to Active
- Click "Save".
4. Managing Notification Settings
To ensure you receive the weekly automated emails from Medallion regarding upcoming expirations, verify your notification settings.
- From your profile, click on the “Email Preferences” tab.
- Make sure all settings are toggled On.
If you run into any issues with Medallion when trying to update your licenses, you can use the Licensure Renewal & Update Intake form to submit your updates instead.
